Individuals: $35Team Members (10 or more): $25 Special $25 rate for Transplant Recipients, Living Donors and Transplant CandidatesFamily Members (4 or more): $25

Donor Families (of deceased donors)

Do not register online! Online registrants will NOT receive t-shirts or buttons with their loved one’s photo! To receive a donor family registration packet, call 800-423-7220 or email familycare@onelegacy.org and include your name, your loved one’s name and your contact information.

TEAM CAPTAINS

You must create your team BEFORE you or any team member can register. Once you have created your team, DON’T forget to register yourself and let your team members know to register. As a team captain, ONLY you will be required to log-in to Active or create an account. Your log-in is your email address. If you have problems logging-in or creating your team, contact Customer Service at 1-877-228-4881.
